Joint Visit / Field Evaluation

Use this tool to document supervised joint visits for field staff. At least one joint visit per discipline per year should be completed to validate clinical competency, documentation, infection control, and compliance with Saintly Home Health policy and payer / accreditation standards.

One form per employee per joint visit. This form should align with the employee’s annual performance evaluation and annual competency checklist.

Joint Visit Assessment – Annual

Complete during the supervised visit while observing care. Submit once per year (or more often, as needed) for each active field clinician and aide.

Do not enter full patient name. Use initials and/or MR#. This form is emailed to the HR / QA inbox for filing in the personnel record.

Rating options: Meets = Competent / safe • Needs Improvement = coaching or action plan needed • Not Observed = not applicable during this visit.
Assessment skills (vital signs, focused assessment, pain, psychosocial, pediatric considerations if applicable)
Clinical interventions performed safely and according to policy / standard of care
Infection prevention & control (hand hygiene, bag technique, PPE, sharps, standard precautions)
Patient / caregiver education (teaching at appropriate level, return demonstration, reinforcement)
Safety practices (fall risk, equipment use, environment, emergency preparedness instructions)
Documentation (timely, accurate, reflects visit, follows payer / OASIS / policy requirements)
Professionalism & communication (respect, confidentiality, collaboration with team / physician)
