Who Qualifies for Medicare Home Health?
We work closely with your physician to confirm Medicare criteria and ensure home health is the right level of care.

You have Medicare (Part A and/or Part B).Traditional Medicare and many Medicare Advantage plans may cover skilled home health services when criteria are met.

You need “skilled” care at home.Services like skilled nursing, physical therapy, or speech therapy ordered by your physician for a specific condition.

You are considered “homebound.”Leaving home requires considerable and taxing effort, help from another person, or use of a walker, wheelchair, or other device.

Your care is under a doctor’s plan of care.Your physician signs home health orders and a plan of care (485) that we follow and update regularly.
- Recent hospitalization, surgery, or skilled nursing facility stay.
- Frequent falls, weakness, or trouble walking safely.
- New or worsening heart failure, COPD, diabetes, or other chronic conditions.
- Complex medication changes or difficulty managing multiple prescriptions.
- Wounds, pressure injuries, or surgical incisions that need skilled care.
- Speech or swallowing challenges following a stroke or illness.
