Who We Help

Who Is Physical Therapy at Home For?

In-home PT is for patients who need skilled therapy but have difficulty safely getting to an outpatient clinic.

  • 1
    After surgery or hospitalization.
Joint replacements, fractures, spine surgery, or medical hospital stays that left you weak or deconditioned.
  • 2
After a stay in rehab or a skilled nursing facility (SNF).
You’re going home but still need therapy to safely move, transfer, and walk in your own environment.
  • 3
    Falls, weakness, or balance problems.
Repeated falls, near-falls, or feeling unsteady when you stand, turn, or walk – especially on stairs or uneven ground.
  • 4
    Chronic conditions affecting mobility.
Arthritis, stroke, neuropathy, heart or lung problems, or other conditions that make moving safely more difficult.
Therapy Goals at Home
Your therapist tailors a plan around your home, your goals, and your safety.
  • Improve strength, balance, and endurance so daily tasks feel easier.
  • Reduce fall risk in and around the home.
  • Teach safe ways to get in/out of bed, chairs, shower, and car.
  • Develop a simple home exercise program you can continue independently.
  • Support a safe transition to outpatient therapy when you’re ready.

Common Questions

Physical Therapy at Home – FAQ

Here are a few of the most common questions we hear about in-home PT.

Does Medicare cover in-home physical therapy?
For patients who meet Medicare home health criteria and have a valid physician plan of care, Medicare typically covers approved in-home PT services. We will verify benefits and explain coverage before visits begin.
Do I need a doctor’s order for PT?
Yes. All home health PT is provided under a physician’s order and plan of care. If you don’t have orders yet, we will help coordinate with your doctor’s office.
How often will the therapist come?
Frequency depends on your needs and the physician’s orders. Many patients start with several visits per week and then gradually taper as they improve and become more independent.
What if I’m able to drive to outpatient PT later?
Great – that’s often the goal. We can help you transition from in-home PT to an outpatient clinic when it’s safe and appropriate, and share progress notes with your new therapy team.
