Registered Nurse (RN)
Position Summary
The Registered Nurse (RN) provides skilled nursing care to home health patients in accordance with the plan of care, physician orders, and Saintly Home Health policies and procedures. The RN coordinates care with the interdisciplinary team, ensures accurate documentation, and promotes patient safety and goal achievement in the home setting.
Essential Duties & Responsibilities
- Performs comprehensive nursing assessments and ongoing reassessments in the home.
- Develops, implements, and updates the nursing plan of care based on patient needs.
- Provides direct skilled nursing interventions (e.g., wound care, IV therapy, injections, catheter care) as ordered.
- Documents all visits, interventions, and communications in the EMR accurately and timely.
- Coordinates with physicians, therapists, aides, and other team members regarding patient status and plan of care.
- Teaches patients and caregivers about disease processes, medications, and self-care techniques.
- Participates in case conferences, QAPI activities, and staff meetings as requested.
- Adheres to infection control, safety, and emergency procedures at all times.
Reporting & Supervision
- Reports to: Clinical Director / Director of Nursing.
- May supervise: LPNs, HHAs, and other designated staff per state regulations and agency policy.
Qualifications
- Current, unrestricted RN license in the state of practice.
- Minimum experience required by agency policy and payer requirements.
- Current CPR certification and TB screening per policy.
- Valid driver’s license, reliable transportation, and required auto insurance (for field staff).
