What You’ll Need Before You Start

Having the following ready will help you finish the onboarding packet faster and prevent delays in scheduling you for visits.

A copy of your professional license or certification (RN, LPN, PT, OT, ST, HHA, MSW, CNA, etc.).

Government-issued photo ID and work authorization (if applicable).

Current CPR/BLS card and any specialty certifications (if required).

Arizona Level One Fingerprint Clearance Card (required for all clinical staff and some administrative roles).

Proof of required immunizations or TB testing (if available).

Contact information for at least two professional references.

Your preferred email and mobile number for schedule and HR updates.

Field Nurses Only: Auto insurance card (must be current and list the employee as an insured driver).

Independent Contractors: Professional liability (malpractice) insurance – must upload proof of coverage.

Resume (required for all clinical staff and independent contractors).
