What You’ll Need Before You Start
Having the following ready will help you finish the onboarding packet faster and prevent delays in scheduling you for visits.

A copy of your professional license or certification
(RN, LPN, PT, OT, ST, HHA, MSW, CNA, etc.).

Government-issued photo ID and work authorization (if applicable).
3
Current CPR/BLS card and any specialty certifications (if required).
4
Arizona Level One Fingerprint Clearance Card
(required for all clinical staff and some administrative roles).

Proof of required immunizations or TB testing (if available).
6
Contact information for at least two professional references.
7
Your preferred email and mobile number for schedule and HR updates.

Field Nurses Only:
Auto insurance card (must be current and list the employee as an insured driver).

Independent Contractors:
Professional liability (malpractice) insurance – must upload proof of coverage.
10
Resume (required for all clinical staff and independent contractors).
